Phyllis Jacquelin Sullivan, 89, passed away on December 29, 2005 in Lake Havasu City, Arizona. She was born on January 10, 1916 in Oak Creek, Colorado to James Hughes and Margaret "Bateman".Phyllis moved to Lake Havasu City, Arizona 11 years ago from Milpitas, California. She was a member of the Wild Rose Chapter of O.E.S. Her family said that she was a very good mother and grandmother.Phyllis is survived by her son Jack E. Sullivan (Marilyn) of Coeur d' Alene, Idaho; daughter Cindy Covillo (Wayne) of Lake Havasu City, Arizona; brother Edwin Hughes (Margaret) of Sun City, Arizona; five grandchildren, 10 great grandchildren and numerous nieces and nephews. She is preceded in death by her husband Tim Sullivan and son Eugene Sullivan.Graveside services will be held at Oak Creek Cemetery in Oak Creek, Colorado on January 5, 2006 at 1:00 P.M.
